LifeX FAQ: Common Questions About LifeX Health Plans

LifeX is available only through select certified brokers like Smart Insurance Strategies. There is no public online sign-up. Call 704-940-6654, text 704-360-0462, or book a call at calendly.com/smartinsurancestrategies. We walk you through the suitability assessment, help you choose a plan, and complete your enrollment with you.


No. LifeX enrollment goes through a LifeX-certified broker. Smart Insurance Strategies is certified, and we also train and certify other brokers.


LifeX coverage always starts on the 1st of the month, as long as your enrollment is complete by the cutoff date the month before.


Your enrollment must be complete no later than the end of the day on the 22nd of the month before. For example, to start November 1, enroll by 11:59 pm on October 22. If you enroll after the 22nd, coverage starts on the 1st of the following month.


It's a short set of eligibility questions (sometimes called knock-out questions) that determines whether you're suitable to work with LifeX. Answer honestly and based on your own situation. Your answers are reviewed by LifeX, not by us.


LifeX Research Corp is a health data and research organization. It offers an employer-sponsored health benefit plan to LifeX Research Associates, who are W-2 employees that voluntarily take part in health data collection, such as wellness activities and surveys.


Yes. Research Associates are W-2 employees of LifeX Research and are paid for completing wellness activities. You'll receive a W-2 at year end, and those payments are taxable.


No. LifeX is a real employer with an ERISA-governed employee benefit plan. The plan is underwritten by Benefit Logistic Captive Insurance Co (NAIC #17633), with BHPI (Benefit Health Plan Inc.) as the third-party administrator (TPA). Read our full blog post, "Is LifeX a Scam?"

Yes. LifeX plans are ACA-compliant and meet Minimum Essential Coverage (MEC).


Most pre-existing conditions are covered. Check the plan documents for details, or ask us about your specific situation.


LifeX plans use the PHCS PPO network. Before you enroll, check whether your doctors are in-network. We're happy to look them up for you.


Three plans, all on the PHCS PPO network: Copay PPO $1,500, VL $1,500 and Major Medical $7,250. Each plan has its own page on our site with full benefits, 2026 rates and the brochure.


Rates depend on your age and who is covered. For example, 2026 monthly rates for ages 18-29, employee only, are $267 (VL $1,500), $369 (Copay PPO $1,500) and $521 (Major Medical $7,250). See each plan page for the full rate table.


Yes. Every plan has Employee + Spouse, Employee + Child(ren) and Family rates. Each spouse or dependent over 18 gets their own login to the Employee Wellness Hub to view their ID card and claims.


Yes. All 2026 LifeX plans include $0 OurLiveDoc telemedicine with unlimited visits for primary care, urgent care and behavioral health. Call 940-548-3362. Preventive care is also $0, with no deductible.


Yes, through ProAct (1-888-250-8032). On the Copay PPO and VL plans, preventive and generic drugs are $0. The Major Medical plan has $0 / $10 / $90 / $110 copays and a $200 GLP-1 copay for qualifying members (not covered for weight loss). Brand and specialty drugs go through patient assistance programs.


Many services require pre-certification, and elective surgery is not covered in the first 90 days. Check your plan brochure for details.


You can change plans during LifeX's annual open enrollment. Open enrollment for the 2027 plan year opens November 1, 2026, and changes take effect January 1. Last year's window ran November 1 to December 5. Contact us early so you don't miss the deadline.


Online in Employee Navigator. LifeX emails you instructions when open enrollment starts. You can update your information, add or remove dependents, change plans, and enroll in or change dental and vision coverage. We're happy to walk you through it.


Your plan renews automatically on January 1 at the new year's rate.


Only if you have a Qualifying Life Event (QLE), such as marriage, a new child, or losing other coverage. LifeX has a QLE form for this. Contact us and we'll help you with it.


You can cancel, but cancellations can't be made retroactive. For example, a request made on October 1 can't be backdated to August 31 for a refund. Current clients can call or text us first at 704-360-0462.
