You need to call Green Imaging at 844-968-4647 to:

1. Make sure they received your order sent by your physician.

2. Use the locator to find a Green Imaging facility.

Please have your name and zip code ready.

The procedure will be scheduled by your Green Imaging Medical Concierge. Then you will receive a voucher for service.

The Voucher Covers:

The procedure fee & The radiologist fee.

Routine/Annual Mammogram: Covered 100%

When you arrive at your appointment, you will not be required to pay a copay or deductible. You will not be responsible for any other bills. You just need to show your Green Imaging Voucher!

You will not: have a co-pay or deductible, and do not need to show

your health benefits card.

Additionally, you will not be responsible for any bills you may receive

in the mail.

This applies to HSA Plans & TDK Plans

Your referring doctor will get your exam results.

If the exam report was not received by you referring doctor then please call: Green Imaging at
